Just looking for a little information

Hi,
I am trying to make a reproduction award card for a Withey King Six Jr. counter dice game. I have 90% of the artwork done, but I need the Manufacturers
name and address at the very bottom of the award card and also there is something printed just above that in the center of the card. Is there anyone out
there that possibly has the King Six Jr. machine with the marquee award card that has the information that I have mentioned? Any help would be greatly
appreciated.
